You can apply for birth & death registration as per the information provided below. The Details of application procedure, registration, etc. are mentioned as under and citizens must read the instructions carefully and proceed accordingly.

 

 
 
 
 
HOW TO GET A BIRTH CERTIFICATE
 
BIRTH AND DEATH SHOULD BE REGISTERED MANDATORILY WITHIN 21 DAYS OF OCCURRENCE OF THE EVENT (Under Section 7, Rule 5(3),of RBD Act 1969.
 
FOR CURRENT REGISTRATION:-Within 21 days of Birth (UNDER SECTION 12).
 
 1.  In case of Central Referral Hospital Birth:-
 
i.      Discharge summary (mother) & neonatal discharge sheet (child) xerox.
ii.     For Nationality:- Father and mothers documents such as copy of Sikkim Subject (COI) or EPIC (Voter I.D Card) or Pass          Port or Residential Certificate for Indian & Sikkimese Origin) Citizenship or Electrol Card of Nepal/Bhutan (for nationality).
iii.    Father and mothers 'Aadhar card'.
iv.    Marriage Certificate or any marriage proof (earlier child’s birth certificate, documents bearing husband name or                          recommendation from councilor or panchayat.
 
2.  In case of home birth or vehicle delivery:-
 
i.          Form No.1, enclosed copy of Immunization Card.
ii.         Notifiers are A.W.W/ ICDS and ASHA too.
iii.        Councillor Recommendation:- Parents name, Date of Birth and Place of Birth must be stated.
iv.        For Nationality: Father and mother documents such are Sikkim Subject copy or COI or EPIC (Voter I.D Card)or Pass              Port or Residential Certificate.
v.         Father and mothers aadhar card and Marriage certificate or any other marriage proof required.
 
 
ALL THE PHOTO COPY OF DOCUMENTS SHOULD BE ATTESTED OR COMPARED FROM THE RESPECTIVE DEPARTMENT.
 

 
HOW TO GET A DEATH CERTIFICATE
 
BIRTH AND DEATH SHOULD BE REGISTERED MANDATORILY WITHIN 21 DAYS OF OCCURRENCE OF THE EVENT (Under Section 7, Rule 5(3),of RBD Act 1969.

i.        CRH death report (original) (copy of Panchayat), bottom portion of MCCD Slip (piece of white paper)original.
ii.        Deceased, spouse & parents voter Id card & aadhar Card.
iii.       Marriage proof like documents bearing husband name or recommendation from councilor or panchayat.
iv.       If unmarried NOC from Councillor or Panchayat.
v.        If widow contact details is mandatory
vi.      Except from spouse or parents whoever come to collect the death certificate have to provide NOC from deceased                    parents or spouse.
vii.      Recipient any I.D proof.
 
 
 2.      In case of home death:-
 i.       Form No.2
ii.       Notifiers are A.W.W/I.C.D.S Centre & Asha too.
iii.      Councillor recommendation:Parents & spouse name,date of death and place of death must be stated.
iv.      Deceased, spouse & parents voter ID card & Aadhar Card.
v.       Marriage proof like documents bearing husband name or recommendation from councilor of panchayat.
vi.      If unmarried NOC from Councillor or Panchayat
vii.     If widow contact details is mandatory
viii.    Except from spouse or parents whoever come to collect the death certificate have to provide NOC  
         from deceased parents or spouse.
ix.      Recipient any I.D proof.
